Job Details Job Location: GREEN - UNIONTOWN, OH 44685 Position Type: Full Time Salary Range: $62,700.00 - $76,700.00 per year Registered Nurse and Licensed Social Worker positions available. Responsibilities The Care Manager is responsible for assessment and care management of long‑term care clients of any age enrolled in waiver programs. Qualifications Must be a Licensed Social Worker (LSW) or Registered Nurse (RN) . Preferred: at least one year of paid clinical experience in home and community based service. Proof of influenza vaccination required.
