Key Responsibilities

• Evaluate patients referred by physicians to assess their physical needs and create individualized treatment plans.
• Implement therapeutic interventions and exercises in the home setting that align with best practices and the physician's plan of care.
• Utilize evidence-based practices to maximize patient outcomes and enhance the overall effectiveness of treatment provided.
• Consult with and educate patients and their caregivers on exercises, strategies for pain management, and recovery techniques.
• Maintain thorough documentation of patient responses and progress in compliance with state regulations and corporate policies.
• Establish and foster strong therapeutic relationships that encourage patients to achieve and surpass their rehabilitation goals.

Patient Population / Client Focus

In this role, you will have the opportunity to work with a diverse population, including orthopedic, pediatric, neurologic, and sports medicine patients. Your expertise will enable you to address a wide range of conditions, guiding patients through recovery while fostering resilience and independence.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• A valid State License in Physical Therapy.
• A minimum of two years of clinical experience in physical therapy, preferably with an orthopedic focus.
• Basic Life Support Certification from the American Heart Association.
• Effective communication skills, both verbal and written, with the ability to build rapport with patients and their families.
• Proficiency in word processing and familiarity with electronic health record (EHR) systems.

Preferred Experience

• Experience in providing home health care services is highly beneficial.
• Demonstrated ability to practice autonomy in evaluating and treating patients in a home-based setting.
• Previous involvement in multidisciplinary care teams for comprehensive patient management.
