Work Environment

This position is based onsite, primarily within our home care facility. You will collaborate with a multidisciplinary team of healthcare professionals to provide personalized treatment plans for patients recovering from orthopedic conditions. Key Responsibilities

• Conduct thorough evaluations of patients, including reviewing medical histories, performing assessments, and documenting findings to determine individualized treatment plans.
• Communicate evaluation results and treatment strategies to patients and their families, actively involving them in goal-setting to foster a collaborative care approach.
• Deliver evidence-based treatment protocols based on each patient's evolving condition, ensuring that care plans are adapted as necessary.
• Work closely with other healthcare professionals, providing insights and modifications to treatment plans based on patient progress observed during sessions.
• Complete timely documentation of patient care, including initial evaluations, daily notes, and discharge plans, ensuring compliance with healthcare regulations.
• Participate in departmental meetings, contribute to the achievement of departmental goals, and engage in outreach activities to promote community understanding of available rehabilitation services.
• Maintain efficient time management and productivity standards, ensuring high-quality care while adhering to scheduled patient visits.
• Provide clinical instruction and mentoring to students, sharing your expertise and knowledge to help cultivate the next generation of healthcare professionals.
• Actively support initiatives that enhance access to care and promote exceptional patient experiences.

Patient Population / Client Focus

As an Orthopedic Physical Therapist, you will work primarily with patients recovering from surgeries, sports injuries, and other musculoskeletal conditions. This role focuses on helping individuals regain independence and improve their quality of life through personalized rehabilitation strategies, exercise programs, and ongoing support. Your efforts will directly contribute to positive patient outcomes and satisfaction.

Qualifications

• Bachelor's degree in Physical Therapy; a Doctorate in Physical Therapy is preferred.
• Current state license to practice as a Physical Therapist in New Jersey.
• American Heart Association's Basic Life Support (BLS) for Healthcare Providers certification is required.
• A strong commitment to patient care and the ability to work independently in a home care environment.

Preferred Experience

Compensation and Benefits
