Additional Unit Specific Information: End of life care assessment and care planning requiring strong comprehensive assessment skills. Strong communication skills and ability to teach patients and caregivers aspects of end-of-life care. Generally, a traveler case manager is assigned a geographic area, not full territory. When they cover a weekend rotation-they would be expected to make visits throughout our service area- Chittenden & Grand Isle Countries. We have had travelers that float.
