Location: Sacramento, CA Job Type: Full-Time (Onsite) Schedule: Monday-Friday, Days Practice Setting: FQHC / Community Health Outpatient Clinic

Job Overview: We are seeking a Family Medicine or Internal Medicine Physician to provide comprehensive primary care in a mission-driven community health setting serving underserved and diverse populations. The ideal candidate is passionate about community healthcare and comfortable managing a steady outpatient volume.

Key Responsibilities:

• Provide comprehensive primary care, including preventive, acute, and chronic disease management
• Perform histories, physical exams, diagnostic workups, and treatment planning
• Order and review labs, imaging, and other diagnostic studies
• Provide pediatric primary care for patients from birth through 19 years
• Provide patient and family education and coordinate referrals
• Manage patient messages, refills, and lab results
• Maintain timely documentation and complete charts within 72 hours
• Participate in approximately 2 telehealth visits daily
• Maintain productivity and clinical quality standards

Qualifications:

• MD or DO from an accredited U.S. or qualifying international medical school
• Board Certified in Family Medicine or Internal Medicine
• Active, unrestricted California medical license
• Active DEA certificate
• Current CPR certification
• FQHC/community health or underserved population experience preferred
• 1-2+ years serving underserved, low-income, or homeless populations preferred
• Pediatric primary care experience strongly preferred
• Familiarity with HRSA, HEDIS, or PCMH quality measures preferred
• EPIC EMR experience highly preferred
