Work Environment\n

This remote position allows you to work from the comfort of your home while conducting therapy sessions via secure telehealth platforms. You will have a Monday to Friday schedule from 9:00 AM to 5:00 PM, with occasional evening shifts required based on patient needs. Key Responsibilities\n

• Conduct thorough mental health assessments of elderly patients, taking into account their unique physical, cognitive, and emotional needs.

• Develop and implement individualized treatment plans that encompass evidence-based interventions tailored to geriatric patients.

• Provide therapeutic modalities, including individual therapy, family therapy, and group therapy sessions, utilizing online therapy software.

• Collaborate with geriatric psychiatrists and an interdisciplinary team of healthcare providers to formulate integrated care strategies.

• Offer crisis intervention support during mental health emergencies involving older adults, ensuring safety and appropriate follow-up care.

• Educate patients and their families on mental health issues, treatment options, and community resources, enhancing their understanding and support networks.

• Maintain meticulous and confidential documentation of patient interactions in compliance with HIPAA regulations.

• Participate actively in team meetings and case consultations to foster shared learning and improve treatment approaches.

Patient Population / Client Focus\n

Our primary focus is on the mental health concerns prevalent in the aging population. Understanding the complex factors that affect this demographic, you will need to approach each case with sensitivity and a patient-centered mindset.

Qualifications\n

• A minimum of a Bachelor's degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current and valid licensure as an LCSW, LCPC, or LMFT in Illinois.

• Ability to demonstrate strong knowledge of geriatric mental health disorders and effective treatment modalities.

• Excellent interpersonal and communication skills with a strong emphasis on an empathetic, patient-centered approach.

• Proficiency in utilizing electronic health records (EHR) systems to document patient interactions and treatment outcomes.
