Per Diem Occupational Therapist (OT)

Make a difference where it matters most— in the patient's home. @Home Therapy is expanding and looking for a licensed Occupational Therapist to provide home health services to adult and geriatric patients in the Elmhurst community. We are seeking a professional who values patient-centered care and enjoys helping individuals improve their functional abilities and quality of life. As a Home Health OT, You May Conduct occupational therapy assessments and evaluations. Identify barriers to safe and independent daily living. Design treatment plans based on each patient's individual needs. Provide training on adaptive equipment and energy conservation. Educate patients and families on fall prevention and home safety. Track patient outcomes and adjust treatment when appropriate. Coordinate with physicians and other healthcare professionals. What Makes This Position Appealing? Flexible availability.
