RN Lung Cancer Navigator

Job Summary The Patient Navigator Nurse serves as the central hub for patients recovering from strokes, cancer, and long‑term illnesses, advocating and coordinating care across multidisciplinary teams. Responsibilities Coordinate written and verbal communication regarding the medical treatment plan among patient, family, physician, and other team members. Communicate and coordinate the treatment plan with internal and external departments and agencies to maintain continuity of patient care. Assist patients in understanding diagnoses, treatment options, and available resources. Review medical documentation to help physicians complete accurate treatment plans. Serve as an essential link between patients and all other care providers. Follow patients through the care continuum, collaborating with care management resources and facilitating appointments for consults and support services (social workers, PT, OT, ST, dietitians, physicians, financial resources, etc.). Collect data, track outcomes, and support strategic planning processes; provide nursing care and guidance from screening to survivorship. Assist patients and family members in finding solutions to treatment‑related problems, providing education, and supporting difficult decisions. Review and update policies related to services provided to enhance professional practice. Serve as a resource for community educational events and staff education. Qualifications Valid nursing license to practice in Louisiana (as defined by the Louisiana State Board of Nursing). American Heart Association BLS certification.
