What Makes This RN Case Manager Opportunity Extraordinary: Lead with Heart: Guide a multidisciplinary team of LPNs, Aides, Social Workers, and Chaplains in creating personalized care experiences Be the Advocate: Champion each patient's unique needs, preferences, and wishes while supporting their loved ones Create Meaningful Connections: Build relationships that matter, providing counseling and emotional support when it's needed most Make Every Day Count: Develop and implement care plans that prioritize quality of life and comfort Tell Their Story: Document each patient's journey with care, capturing progress and precious moments Your Impact Zone: As an RN Case Manager, you'll travel throughout the Canton area, bringing healing and hope directly to patients in their homes, assisted living facilities, long-term care facilities, and hospitals –wherever they feel most comfortable. Required Qualifications: Active RN License Valid Driver's License 1+ years hospice experience Preferred Qualifications: BLS Certification Hospice and Palliative Medicine background Ready to Answer the Call?
