Remote RN PCS Assessor - Medicaid Home Care (NC)

Vaya Health is seeking a Registered Nurse to perform needs-based assessments for Medicaid beneficiaries in designated North Carolina counties. The PCS Assessor will schedule and conduct telephonic and face-to-face assessments in home and community settings, coordinate with healthcare professionals, and submit SARs for PCS providers. The RN must maintain NC residency or live within 40 miles of the NC border and hold an active NC RN license, with experience in home care and ADL/IADL support
