Comunilife is seeking a dedicated Case Manager for its congregate program in the Bronx. You will provide comprehensive case management, develop integrated recovery plans, and coordinate services to assist clients and families. The role requires a bachelor’s or related degree with relevant experience, plus the ability to conduct monthly home visits and coordinate with healthcare and social services.
