The mission of this role Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on. Objectives Carry a full, high-quality caseload: ramp to ~30 productivity points/week at target visit quality. Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows. What you'll own Deliver skilled PTA visits in the home under the supervising PT’s plan of care. Execute treatment plans with a focus on functional mobility and measurable progress: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention. Set/track objective progress within PTA scope and communicate updates/concerns to the supervising PT. Drive multidisciplinary coordination: elevate changes in condition, align on orders, and keep the care team moving. Coach patients and caregivers with practical home programs that actually get done. Document promptly and thoroughly — defensible, efficient, and consistent. Requirements Active Michigan PTA license (in good standing) Reliable transportation, valid driver's license, auto insurance Home health experience strongly preferred Able to carry/transport basic therapy equipment as needed Competency in core home health PTA scope Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations Strengthening, functional mobility, and home safety assessment/modification education Patient/caregiver coaching with practical home programs Our guarantee Full-time target: ~30 productivity points/week ; clinicians earn more for exceeding it.
