Physical Therapy Assistant -Take Your Therapist Home

As a Physical Therapy Assistant in this role, you will become an integral teammate in our innovative Take Your Therapist Home (TYTH) program. You will provide home health therapy services in conjunction with skilled nursing facility (SNF) based therapy services, supporting patient continuity of care across the health care continuum. Scope This hybrid role focuses on the greater Dublin area, with a steady referral source from multiple skilled nursing facilities. Home health geographic area surrounds our Shorepointe facility at 26001 E. Jeffereson St., Clarksburg, MI 48081. You will deliver services per visit (home health) and hourly (SNF). Schedule Monday – Friday, full time. Required Qualifications Successful completion of an accredited Physical Therapy Assistant program. Licensure as required by the State of Michigan. Valid driver’s license. Skills and Abilities Ability to exercise professional judgement and initiative when analyzing problems and recommending solutions. Initiative and flexibility in the work environment. Ability to react effectively to the needs of patients and their families. Physical ability to bend, position, transfer and lift patients weighing from 80 pounds to over 300 pounds, without restriction, using clinically recommended lifting devices. Why Rivetus?
