Care Manager MSW

$36 - $56.84 per hour

Description Under supervision of Care Management Manager, works as a catalyst to promote the following: coordination of care, continuity of care, achievement of consistent and/or expected psychosocial outcomes, patient advocacy and efficient resource management. Coordinates the care for complex designated patients from pre‑admission to immediate post‑discharge. Required Qualifications Master's Degree in Social Work or related field Upon hire: Washington Clinical Independent Social Worker Associate License 3 years of Social services experience with a health care related organization; experience should include a minimum of 2 years in acute care / care coordination. Pay Range: $36.00 - $56.84 The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
