About Attentive Care

Attentive Care is a community-based healthcare organization dedicated to helping individuals with cognitive decline live safely and independently at home. We work closely with patients and families to provide compassionate coordination and meaningful support.

Position Overview

We are seeking a compassionate and detail-oriented Service Coordinator to oversee a caseload of approximately 25 patients participating in the NHTD program. The individuals served are living with dementia or cognitive decline and require consistent coordination, advocacy, and support. This role offers meaningful, hands-on experience within a collaborative and supportive healthcare environment.

Responsibilities

  • Conduct regular phone check-ins with families and caregivers
  • De-escalate and resolve concerns with service providers
  • Schedule therapies and transportation
  • Complete renewal paperwork and incident reports
  • Conduct in-home visits once per quarter throughout the five boroughs

Training & Schedule

  • 4–6 weeks of paid, in-person training in Brooklyn
  • Hybrid schedule after training (remote work + quarterly home visits)

Qualifications

  • Associate’s or Bachelor’s degree  required
  • Conversational Spanish required 
  • Strong communication and organizational skills
  • Related healthcare or human services experience is a plus, but not required
