Job Details: Schedule: M-F 8:00 am-4:30 pm Location: Marion, IL Job Type: Contract Contract Length: 13 weeks Pay Range: $40/hr- $42/hr Pay offered is based on experience, expertise, credentialing, and education.

Duties:

• Perform comprehensive patient evaluations and develop individualized plans of care
• Provide skilled physical therapy services in patients' homes
• Conduct Start of Care (SOC), Resumption of Care (ROC), recertifications, and discharge visits
• Monitor patient progress and adjust treatment plans as needed
• Educate patients and caregivers on exercises, safety, and ongoing care
• Maintain timely and accurate clinical documentation in compliance with regulations
• Collaborate with physicians, nurses, and interdisciplinary team members
• Ensure compliance with Medicare guidelines and home health regulations
