Job Summary:

The Case Management Assistant supports care coordination and discharge planning processes for ambulatory patients. This role ensures smooth transitions of care by collaborating with healthcare professionals and managing administrative tasks efficiently.

Responsibilities:

• 
  Coordinate activities related to patient discharges and transitions across the care continuum.
  
  
• 
  Partner with Case Managers and Social Workers to assess patient needs and communicate care plans.
  
  
• 
  Manage high-volume administrative tasks, including faxing, voicemail management, and data entry.
  
  
• 
  Prepare and facilitate patient transfers to post-acute facilities, ensuring all documentation is complete.
  
  
• 
  Maintain accurate documentation within Epic and other EMR systems.
  
  
• 
  Escalate barriers to discharge and participate in daily team huddles to promote seamless workflow.
  
  

Qualifications:

• 
  Hold a high school diploma or equivalent with at least 1 year of relevant healthcare experience.
  
  
• 
  Demonstrate proficiency with Epic EHR and utilization review transmission.
  
  
• 
  Possess strong written and verbal communication skills.
  
  
• 
  Exhibit excellent time management and organizational abilities.
  
  
• 
  Capable of working independently in a fast-paced environment with shifting priorities.
  
  
• 
  Be able to work PST hours, including rotating weekends and holidays.
