Case Manager (Supports Planner)

Job Category Supports Planning Services (SPS) Service Coordination, Inc. is looking for Supports Planners with a passion for serving their community. We offer competitive pay with a base salary range of $63,107-$78,264 for all incoming case managers, based on education, relevant case management experience, and certifications. Responsibilities include establishing positive working relationships, developing person‑centered plans of service, ensuring compliance with state‑mandated requirements, and advocating for clients’ needs. Essential Duties Advocate for people served, provide resource information, educate clients and their support system toward self‑determination. Collect eligibility information, obtain documentation, and develop/update person‑centered plans of service (POS). Engage in person‑centered planning process, collaborate on POS creation and preparation. Research and coordinate resources to achieve outcomes specified in POS. Monitor POS and conduct follow‑up activities including on‑site visits, progress assessment, and compliance with regulations. Adhere to SCI policies, meet performance benchmarks, maintain HIPAA records, and utilize tracking tools. Experience: Preferably in working with people with disabilities and with person‑centered planning. Physical Demands Office‑based work requiring ≥ 95% of time stationary; periodic mobile tasks up to 25 pounds; must operate a motor vehicle; good close vision.
