Registered Nurse Supervisor - Evening Shift

Job Summary:

Constant Care Health Services LLC is seeking a dedicated and compassionate Registered Nurse Supervisor to lead our evening shift team in The Bronx, New York. As a Registered Nurse Supervisor, you will play a vital role in ensuring the highest level of care for our patients while promoting a positive and supportive work environment for our home care professionals.

About the Role:

As a Registered Nurse Supervisor, you will be responsible for supervising a team of home care professionals, providing guidance and support to ensure the delivery of exceptional patient care. You will also be responsible for:

• Coordinating patient care activities to ensure that all patients receive the highest level of care and support

• Supervising and mentoring our home care professionals to promote professional growth and development

• Maintaining effective communication with patients, families, and healthcare providers to ensure that all parties are informed of patient status and treatment plans

• Identifying and addressing any concerns or issues related to patient care or staff performance

Responsibilities:

• Oversee patient care within the evening shift, ensuring that all patients receive timely and effective care
• Supervise and evaluate the performance of home care professionals, providing constructive feedback and coaching as needed
• Collaborate with other healthcare professionals to ensure that patients receive comprehensive and coordinated care
• Maintain accurate and timely records of patient care and staff performance

Requirements:

• Current RN licensure in the State of New York

• Minimum 2+ years of experience as a Registered Nurse, with experience in home care or long-term care preferred

• Excellent communication and leadership skills , with the ability to work effectively with patients, families, and healthcare professionals

• Strong organizational and time management skills , with the ability to prioritize tasks and manage multiple responsibilities
