Role Description This full-time, on-site Physical Therapist role is based in the Greater Myrtle Beach Area and focuses on providing high-quality rehabilitative care to senior residents within partner communities. The Physical Therapist will assess functional abilities, develop individualized treatment plans, and deliver evidence-based interventions to improve mobility, strength, balance, and independence. Daily responsibilities include coordinating care with community staff, documenting clinical progress, educating residents and caregivers on exercise programs and safety, and participating in interdisciplinary meetings to optimize outcomes. The role also involves identifying emerging clinical needs, contributing to fall prevention and wellness initiatives, and supporting training for community staff on rehabilitation-related topics. Qualifications Strong foundation in Physical Therapy and Rehabilitation, with the ability to design and implement individualized treatment plans. Competence in Medicine-related concepts relevant to geriatric care and chronic condition management. Demonstrated skills in Patient Care, including assessment, monitoring, and ongoing evaluation of functional status. Experience or aptitude in Home Care or community-based care models, supporting residents in their living environment. Current licensure or eligibility for licensure as a Physical Therapist in the state of practice. Experience working with older adults or in senior living settings preferred. Effective communication and teamwork skills, with the ability to collaborate with interdisciplinary staff and families. Ability to work on-site in the Greater Myrtle Beach Area, manage a clinical caseload, and maintain accurate documentation in accordance with regulatory and organizational standards. #J-18808-Ljbffr
