Provide ongoing support, guidance, and advocacy for clients throughout their service journey. Maintain accurate, detailed, and confidential client records using our digital case management system. Regularly communicate with clients via phone, video conferencing, and secure messaging to ensure continuity of care. Collaborate with other professionals, including therapists, medical practitioners, and government agencies. Stay informed about available social support programs, policies, and best practices. Prepare reports on client progress, outcomes, and identified trends. Requirements Bachelor's degree in Social Work, Psychology, Sociology, or a related field. Minimum of 3 years of experience in case management or social support services. Proven ability to work independently and manage a caseload effectively in a remote setting. Excellent verbal and written communication skills, with strong active listening abilities. Proficiency in using digital case management software and standard office applications (e.g., Microsoft Office Suite, Zoom).
