Remote Clinical Case Manager - Long-Term Care & Social Work
CVS Health
CVS Health in Sterling Heights, MI is hiring a Coordinator of Care to support members with long-term rehabilitation and psychosocial needs. You will coordinate all case management and quality‑of‑life activities across the care continuum, linking home care, community resources and providers.
The role requires collaboration with clinicians and community partners, travel up to 50% in Wayne and Macomb Counties, and a Monday–Friday 8:00AM–5:00PM schedule.
