Remote Clinical Case Manager
CVS Health
CVS Health is seeking a qualified social work professional to coordinate care for members with long-term rehabilitation and psychosocial needs. This remote role includes travel to Wayne and Macomb Counties to implement and coordinate case management services.
You will assess and plan care, collaborate with medical teams, and develop programs to reduce hospital admissions and improve quality of life for members and communities.
