Nurse Practitioner - Riverside

Job Summary SCAN is an experienced nonprofit health organization serving seniors across the Southwest United States. We are seeking an experienced and motivated Advanced Practice Clinician (APC) based in Riverside, CA, to provide high‑quality patient care in Skilled Nursing Facilities, collaborating with physicians and other healthcare professionals to ensure best medical standards. Responsibilities Patient Care: Conduct comprehensive patient assessments, including medical history, physical exams, and diagnostic tests; develop and implement treatment plans, prescribe medications and therapies; provide patient education and counseling on health maintenance, disease prevention, and treatment options. Collaboration: Work closely with physicians, nurses, and other healthcare professionals to coordinate comprehensive care; participate in interdisciplinary team meetings and case conferences to discuss patient care and treatment plans. Quality Improvement: Monitor and evaluate patient outcomes to identify areas for improvement in care delivery; participate in quality improvement initiatives and research projects. Documentation: Maintain accurate and detailed patient records, including medical histories, treatment plans, and progress notes; ensure compliance with all regulatory and documentation standards. Professional Development: Keep current with advancements in medical knowledge and clinical practices through ongoing education and training; participate in professional organizations and conferences. Other duties as assigned; actively support SCAN’s vision and goals. Qualifications Nursing degree; completion of an accredited Advanced Practice Clinician program. Certification as an Advanced Practice Clinician (preferred). 3–5 years of related experience (preferred). Technical expertise: advanced analytical skills. Business Insight: in‑depth understanding of the business. Clinical expertise: strong knowledge of clinical practices and procedures; ability to diagnose and treat a wide range of medical conditions in Skilled Nursing Facilities. Critical thinking: analyze complex patient data and make informed clinical decisions. Teamwork: collaborate with a multidisciplinary healthcare team; strong interpersonal skills to build effective relationships. Technology proficiency: proficiency with electronic health record (EHR) systems and other clinical software; use technology to enhance patient care and documentation. Time management: manage a busy caseload and prioritize tasks effectively; strong organizational skills for timely patient care. Attention to detail: maintain accurate patient records; meticulous in following clinical protocols and procedures.
