Physical Therapist (PT) - Home Health

Choice Health at Home is seeking a compassionate and motivated Physical Therapist (PT) to join our growing Home Health team. If you're passionate about helping patients regain mobility, improve function, and maintain independence in the comfort of their own homes, we'd love to hear from you.

As a Home Health Physical Therapist, you'll work independently while collaborating with an interdisciplinary team to deliver exceptional, patient-centered care.

Responsibilities

• Perform comprehensive physical therapy evaluations and establish individualized treatment plans.
• Assess patient needs, identify functional limitations, and determine appropriate physical therapy interventions.
• Develop and implement treatment programs to improve mobility, relieve pain, restore function, and prevent disability.
• Monitor patient progress and modify treatment plans to achieve optimal outcomes.
• Educate patients, families, and caregivers on treatment plans, home exercise programs, and safety techniques.
• Maintain accurate, timely, and compliant clinical documentation.
• Follow infection control and universal precaution guidelines.
• Communicate effectively with physicians, nurses, and other members of the healthcare team.
• Support the mission, values, and standards of Choice Health at Home.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current, unrestricted Physical Therapist (PT) license.
• One (1) year of Home Health experience preferred.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Current automobile insurance.
• HomeCare HomeBase experience preferred.

Physical Requirements

• Ability to stand, walk, bend, kneel, squat, climb, and sit for extended periods.
• Ability to lift, carry, push, or pull up to 100 pounds with or without reasonable accommodation.
