Position: Registered Nurse (RN) - Home Health

Shift: Day 5x8-Hour (08:00 AM - 05:00 PM)

Description:

The Registered Nurse (RN) - Home Health position involves coordinating and directing the provision of home nursing care according to physician orders and agency policies. The RN will apply the nursing process competently and is responsible to the Clinical Manager. This role includes direct supervision and evaluation of Home Health Aides as well as Licensed Practical Nurses/Licensed Vocational Nurses.

Key Responsibilities:

• Provide comprehensive home nursing care as per physician's orders.
• Supervise and evaluate the performance of Home Health Aides and LPN/LVN staff.
• Ensure compliance with agency policies and procedures.
• Complete documentation within 24 hours of patient appointments.
• Participate in periodic office meetings and staff training.

Travel Requirements:

This role requires frequent home visits and the candidate must be willing to travel up to 75 miles from the assigned clinic. Mileage will be reimbursed at the IRS standard rate. Candidates are expected to have reliable transportation, a current driver's license, and agency-required liability insurance.

Productivity Expectations:

• Average number of patients per day: 5-10
• Weekly productivity points expected: 30 points
• Each patient visit and activity (e.g., documentation, travel) holds a different point value.

Qualifications:

• Minimum of 1 year of experience in Adult Home Health or Hospice care.
• Current CPR and BLS certifications required.
• Proficiency in HomeCare HomeBase (HCHB) and Oasis is necessary.
• Must have a valid nursing license in hand.
• Prior travel experience is preferred.

Schedule:

The typical schedule is Monday through Friday, 8:00 AM to 5:00 PM. Weekend rotation and on-call responsibilities vary depending on the location and profession.

Location:

Candidates may be expected to serve the following areas: Hull, Halifax, Kingston, Plymouth, Pembroke, Marion, Mattapoisett, and Wareham, MA.

Additional Information:

• Must be open to occasional on-call rotations and weekend work.
• No guaranteed hours are provided; schedule may vary based on patient census.
• Local candidates are accepted, with similar compensation rates.
