Locations Woburn, Burlington, Bedford, Lexington, Winchester, Stoneham Responsibilities Provide outpatient rehabilitation services in an in‑home setting for the older adult population. Evaluate and treat patients in the most functional environment which is the patient’s home. Assess the need for, develop, and deliver physical therapy programs to facilitate rehabilitation. Identify the need for and make referrals to other disciplines to address comprehensive needs of patients. Implement individualized therapy programs designed to restore, reinforce, and enhance performance. Instruct, educate and train patients and caregivers in skills and functions essential for promoting independence and productivity. Direct activities to promote and maintain health. Provide a flexible schedule and work environment in various settings. Supervise Physical Therapy Assistants, temporary licensees, students and support personnel in accordance with state licensure requirements and professional standards. Maintain adherence to department and patient treatment schedule, and complete timely clinical and billing documentation requirements. Attend and contribute to patient care, staffing conferences and other related meetings. Consult with other members of the rehabilitation team to select the most appropriate therapeutic regimen. Adapt program and/or plan of care according to the needs of the individual patient. Promote a safe environment and enforce elimination of fire and safety hazards. Order supplies and equipment as necessary. Clean, maintain, identify, and report the need for repair of equipment as necessary. Ensure patients and families receive the highest quality service in a caring and compassionate atmosphere. Screen, examine and evaluate patients, including history, systems review, and application of appropriate tests and measures. Develop appropriate treatment goals and methods in collaboration with the patient and caregivers, implement the treatment plan, and complete all related documentation. Provide services including therapeutic exercise, functional training, manual therapy, airway clearance techniques, integumentary repair, electrotherapy, physical agent and mechanical modalities, health and wellness, education on chronic disease management, falls assessment and interventions, case management in the home and community. Design, fabricate, fit, and/or apply adaptive devices and train patients in their use. Select appropriate activities suited to the individual's physical and mental capacity, intelligence level and interest. Instruct patient in skills and techniques required for participation in mobility activities. Perform routine reexamination as needed to modify/progress plan of treatment. Incorporate health and wellness programming into treatment plans for patients. Perform home and community mobility assessments and provide recommendations for safe mobility. Perform other related duties as required. Qualifications Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility. Master’s degree in Physical Therapy. Master’s degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy. Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience. Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent. Valid driver’s license or reliable, independent transportation to and from patients’ residence. Salary and Bonus Posted Salary Range: USD $47.00 - USD $63.00 /Hr. Bonus: USD $5,000.00 #J-18808-Ljbffr
