Overview Physical Therapist – Mobile Outpatient Therapy. Bonus: $5,000. Locations: Woburn, Burlington, Bedford, Lexington, Winchester, Stoneham. Patients include residents of assisted living facilities (ALF) and residential community areas. Responsibilities Provide outpatient rehabilitation services in the home setting for older adults. Evaluate and treat patients in the most functional environment – patient’s residence or independent living facility. Develop and deliver individualized physical therapy programs to restore, reinforce, and enhance performance. Instruct and train patients and caregivers in skills essential for independence. Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel as required. Adhere to department and patient treatment schedules, complete clinical and billing documentation, and attend related meetings. Contribute to patient care, staffing conferences, and other team meetings. Make referrals to other disciplines and agencies to address comprehensive patient needs. Collaborate with rehabilitation team members to select appropriate therapeutic regimens. Adapt program and/or plan of care based on patient needs. Promote a safe environment and enforce elimination of safety hazards. Order supplies and equipment as necessary; maintain and report equipment repairs. Ensure patients and families receive high‑quality, compassionate service. Screen, examine, and evaluate patients; synthesize findings to establish diagnosis, identify impairments, set goals, and plan care. Implement therapeutic techniques, including exercise, functional training, manual therapy, airway clearance, modality use, education on chronic disease management, fall risk assessment, and home/community mobility assessment. Design, fabricate, fit, or apply adaptive devices and train patients in their use. Instruct patient skills and techniques for mobility activities; evaluate progress, attitudes, and behavior. Perform routine reexamination to modify or progress treatment plans. Incorporate health and wellness programming into treatment plans. Perform home and community mobility assessments and provide safe mobility recommendations. Perform other related duties as required. Qualifications Valid Physical Therapy license in the state(s) of practice or proof of license eligibility. Master's degree in Physical Therapy or a related field with a Bachelor's degree in Physical Therapy; or Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience. Physical Therapy degree earned outside the U.S. must be evaluated as the U.S. equivalent. Valid driver's license or reliable, independent transportation to patients’ residences.
