Licensed Practical Nurse (LPN) - Skilled Nursing

The Skilled Nursing LPN provides resident-centered nursing care in a skilled nursing facility, supporting post-acute, long-term, and chronic condition management. The role emphasizes monitoring, comfort, and care coordination with the care team.

Under supervision of an RN, the LPN administers medications, provides wound care, and supports activities of daily living to maintain residents' safety and well-being.

Responsibilities

• Assess residents' conditions, monitor vital signs, and report changes promptly.
• Administer medications and provide basic wound care and dressing changes.
• Assist with activities of daily living and mobility as appropriate.
• Support care planning and document progress and events in the EHR.
• Collaborate with RNs, CNAs, physicians, and therapists to implement care plans.
• Provide patient and family education on health concerns and post-discharge care.
• Monitor for adverse drug reactions and safety concerns; respond to emergencies per facility policy.
• Assist with admission, transfer, and discharge processes as part of the care team.

Qualifications

Graduate of an accredited Licensed Practical Nurse program.

Current LPN license in good standing; state-specific requirements apply.

• Basic Life Support (BLS) certification.
• IV therapy certification where allowed by state policy.

Experience in skilled nursing or long-term care; strong assessment, documentation, and teamwork skills.

Experience

• Direct resident care in a skilled nursing facility or long-term care setting.
• Vital signs monitoring and routine nursing procedures.
• Medication administration and documentation in the EHR.
• Collaborative care planning with RNs, CNAs, and physicians.

Preferred

• Experience with wound care and post-acute care transitions.
• Familiarity with facility policies, payer requirements, and quality metrics.

Work Environment

Work in a skilled nursing facility or long-term care setting with a multidisciplinary team; steady pace, with on-site supervision and rotating shifts.

Documentation and care coordination occur in the facility's electronic health record; emphasis on resident comfort and safety.
