Duties and Responsibilities The Physical Therapist/PT will assess referred patients requiring rehabilitation services, providing direct patient care, assessing medical condition and functional capabilities, and determining potential for rehabilitation. The PT will establish and administer a treatment program with specific goals based on the patient's capacity and tolerance under physician direction, set realistic goals, document patient status, and educate patients and families on appropriate physical therapy methods. Develops a plan of care for each physical therapy patient. Provides skilled physical therapy services/interventions per physician orders. Assesses patient needs, plans, evaluates, and modifies care to meet goals of physical therapy interventions. Collaborates with all disciplines to plan and evaluate team goals for each patient.
