Licensed Social Worker (LSW) – Hospital | Days | MinnesotaWe are seeking an experienced Licensed Social Worker (LSW) for a hospital-based contract opportunity in Minnesota.Responsibilities:Provide psychosocial support to patients and families during illness, injury, and hospitalization.Assist with discharge planning and coordination of care with appropriate next-level services.Assess social, emotional, financial, and family factors impacting patient care and recovery.Coordinate referrals to skilled nursing facilities, assisted living, hospice, group homes, and community resources.Support patients and families with complex social needs, including mental health, abuse concerns, guardianship, and end-of-life planning.Collaborate with the healthcare team to ensure safe and timely patient transitions.Requirements:Minimum 2 years of hospital social work experience.Discharge planning experience required.Experience working with all patient populations.Active Minnesota LSW License (must provide documentation).Epic EMR experience required.Current skills checklist and supervisory references required.If you are an experienced hospital Social Worker looking for a rewarding opportunity, we'd love to connect!
