Key Responsibilities

• Conduct thorough evaluations of residents to accurately assess their physical needs and capabilities.
• Design and execute individualized treatment plans that focus on enhancing strength, mobility, and functional independence.
• Implement a variety of therapeutic techniques, including therapeutic exercises and manual therapy, tailored to address each resident's unique challenges.
• Track and document patient progress meticulously, making necessary adjustments to treatment plans based on evolving needs and recovery milestones.
• Engage in regular collaboration with an interdisciplinary team to develop comprehensive care strategies that encompass all aspects of a patient's health.
• Educate residents and their families on home exercise routines and effective strategies to maintain and promote physical health outside of therapy sessions.
• Maintain strict compliance with all state and federal regulations as well as professionalism standards within the therapeutic environment.

Patient Population / Client Focus

Your primary focus will be on residents within our rehabilitation facility, predominantly servicing the geriatric population. Many patients may be recovering from surgery or managing chronic conditions that affect their mobility and overall physical wellness. You will have the opportunity to engage with patients on a personal level, understanding their unique situations and ensuring that your therapeutic approaches align closely with their goals and lifestyle.

Qualifications

• Possession of a current and valid Physical Therapist license in the state of New York.
• A Bachelor's Degree in Physical Therapy is required; however, a Master's Degree or Doctorate in Physical Therapy from an accredited institution is highly preferred.
• Commitment to ongoing education and staying current with trends and best practices in physical therapy.
• Exceptional communication, organization, and interpersonal skills that enable effective collaboration within a team environment.
• A strong commitment to providing compassionate care tailored to individual resident needs.

Preferred Experience

• At least two years of experience in a similar role, preferably within a skilled nursing facility or focusing on geriatric care.
• Hands-on expertise in rehabilitation practices and familiarity with treatment software that tracks resident progress and therapy plans.
• Previous exposure to interdisciplinary collaboration, especially within rehabilitation settings.

Compensation and Benefits
