Mindify Wellness and Care is seeking a compassionate, skilled, and client-centered Mental Health Therapist to provide individual therapy services to clients with a variety of mental health diagnoses. Working as part of a collaborative behavioral health team, the therapist will support clients in achieving their treatment goals through evidence-based interventions, care coordination, advocacy, and ongoing support.

This position provides the opportunity to deliver care through a hybrid model, combining telehealth services with in-person clinical care at adult day care centers in the Perth Amboy and South Amboy, New Jersey areas.

Essential Duties and Responsibilities

• Provide individual therapy to clients with a variety of mental health needs, including depression, anxiety, trauma, substance use disorders, and other behavioral health concerns.
• Develop and implement individualized treatment plans tailored to each client's needs, strengths, and goals.
• Conduct comprehensive clinical assessments and evaluate client progress throughout treatment.
• Coordinate care with other providers, including psychiatrists, psychiatric nurse practitioners, psychologists, primary care providers, and other healthcare professionals.
• Advocate for clients and assist with identifying appropriate community resources and support services.
• Provide education, guidance, and emotional support to clients and their families.
• Maintain accurate, timely, and compliant clinical documentation.
• Participate in multidisciplinary team meetings and treatment planning discussions.
• Engage in ongoing professional development and training activities.
• Deliver services through a hybrid care model, conducting sessions both virtually (telehealth/video) and onsite as needed. Ability to seamlessly transition between service modalities is required.

Qualifications and Requirements

• Master's degree in Social Work, Counseling, or a related behavioral health field from an accredited program.
• Active New Jersey licensure as an LCSW, LSW, LAC, LPC, or equivalent behavioral health license required.
• Minimum of two (2) years of experience providing mental health services preferred.
• Bilingual in Spanish and English required.
• Strong clinical assessment, therapeutic, and treatment planning skills.
• Excellent communication and interpersonal skills.
• Ability to work independently while collaborating effectively within a multidisciplinary team.
• Commitment to delivering culturally responsive, equitable, and client-centered care.

Work Location

• Hybrid position serving clients in the Perth Amboy and South Amboy, New Jersey areas.
• Combination of remote telehealth and in-person clinical services.

Compensation

$55,000–$75,000 annually , depending on licensure, experience, and qualifications.
