Community Health Advocate & Care Navigator
Medix™
Medix is seeking a compassionate Community Health Worker to join our care management team. You will build relationships with community partners, provide outreach to members, and support individuals by connecting them with healthcare, housing, and social resources. The ideal candidate has strong case management experience, excellent documentation skills, and a passion for serving underserved populations. Schedule includes 40 hours/week, Monday–Friday, 8:00 AM–5:00 PM, with occasional evenings and #J-18808-Ljbffr Medix™
