Overview Contra Costa County offers a full spectrum of health-related services under one organizational structure. The Contra Costa Health Department has over 5,200 employees in nine divisions: Office of the Director, Contra Costa Regional Medical Center, Health Centers, Public Health, Behavioral Health, Emergency Management System, Environmental Health, Hazardous Materials, Health, Housing and Homeless, and Contra Costa Health Plan. This position is for Medical Social Worker II with opportunities in inpatient and outpatient settings at Contra Costa Regional Medical Center, Health Centers, Behavioral Health programs, and Public Health programs. Responsibilities Interview families and clients/participants with intensive care management to develop a comprehensive assessment of needs. Interagency coordination and collaboration. Attend and actively participate in interdisciplinary rounds or team meetings to review client needs and plans. Review and maintain chart documentation that is timely, clear, relevant, and thorough. Offer consultation and recommendations regarding patient care and discharge planning to medical and ancillary teams. Communicate and provide outreach to clients with increased psychosocial needs, including crisis intervention and addressing basic needs (food, housing). Experience: Two (2) years of full-time experience providing counseling and/or social work services to medical patients. Substitution for Education and Experience: Possession of a Baccalaureate Degree from an accredited college or university with a major in Social Work, Psychology, Counseling, or a closely related behavioral science field and either: a) two years of full-time or equivalent experience as a Medical Social Worker I in Contra Costa County; or b) three years of full-time or equivalent experience providing counseling and/or social work services to medical patients. Substitution for Academic Major: One (1) additional year of qualifying experience may substitute for the required major. Desirable Qualifications: Knowledge of multisystem resources (e.g., Regional Center of the East Bay, School District, Behavioral Health, IHSS), knowledge of conservatorship/guardianship and related court documents, post-master’s experience with medical patients, experience with mental health and substance use disorders, and knowledge of Medicare/Medi-Cal.
