Clinical Social Worker II/III - Crisis & Discharge Lead

The Clinical Social Worker intervenes with patients who have complex psychosocial needs, assists with eligibility determination for social programs and funding sources, offers crisis intervention to pre‑/post‑Kidney Transplant patients and families, and coordinates a discharge plan of care for high‑risk patient populations. The CSW collaborates with Case Managers, Nursing Staff, Medical Staff and ancillary departments to support a timely, safe discharge and to meet continuing care needs within fiscal realities. The CSW participates in department education and practice improvement activities, works in a collaborative practice model, facilitates care coordination with the multidisciplinary transplant team, optimizes patient outcomes, and advances timely, medically‑ready discharge. Responsibilities Conduct psychosocial assessment and intervention based on preliminary risk screening. Provide crisis intervention, family counseling, and support for emotional, social, and financial consequences of illness. Serve as primary care manager for selected patients, collaborating with case managers to manage resources and transition planning from admission to discharge. Lead multidisciplinary rounds, monitor discharge milestones, and address barriers to timely discharge. Reassess patients and adjust discharge plans in real‑time as needed. Maintain accurate documentation in APeX screens, patient records, and medical records. Collaborate with psychiatry and other teams on psychiatric holds, mental health, substance abuse issues, and crisis interventions. Educate patients, families, physicians, and community about post‑acute options, end‑of‑life issues, and advance directives. Participate in quality improvement activities, program planning, and departmental coverage as needed. Perform clinical supervision for CSW II staff and provide oversight for delegated activities. Comply with UCSF policies, documentation standards, and timely submission of required statistical data. Engage in continuous learning, including departmental, hospital, and community in‑services. Qualifications Clinical Social Worker II: Master’s degree from a CSWE‑accredited school of Social Work. Experience (internship/career) in a healthcare and/or hospital setting preferred. Knowledge of community resources, medical and psychological sequelae, and relevant patient population. Strong written and oral communication, diplomacy, organization, and analytic skills. Self‑directed, assertive, creative problem‑solver in a high‑volume environment. Ability to work effectively with interdisciplinary teams. Basic pager, telephone, and computer skills. Flexibility to work at all UCSF Medical Center locations. Clinical Social Worker III: All qualifications for Clinical Social Worker II. Licensed in Clinical Social Work in the state of California (Board of Behavioral Sciences). Three years of experience in a healthcare setting, preferably hospital.
