Job Title: Clinical Nurse Profession: Registered Nurse Specialty: Outpatient Duration: 9 months Shift: 8 AM - 4 PM Hours per Shift: 37.5 Experience: Minimum 2 years of recent hospital outpatient RN experience License: NYS RN License Certifications: BLS, ACLS Must-Have: 2 years of ambulatory experience Description: The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care. Educates patients and caregivers about their plan of care, transition of care, promotion of health, and prevention of disease. Communicates effectively and professionally with patients, family, and all members of the Interdisciplinary Patient Care Team. Manages assignments within the Care Delivery Model of Modified Primary Nursing. Role models the tenets of Relationship Centered Care through facilitation of patient care actions. Considers patient values, preferences, and expressed needs in all aspects of care. Establishes and maintains a therapeutic relationship with the patient and family. Initiates service recovery for patients and family members who have concerns as needed. Demonstrates caring, respect, compassion, empathy, and active listening through dialogue and actions. Conducts Purposeful Hourly Rounding on assigned patients addressing core care needs. Practices safe and efficient patient-centered care, identifying barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. Collaborates with interdisciplinary teams to create and implement quality improvement projects and evidence-based practices. Implements principles of high reliability to identify and avoid high-risk behaviors. Documents nursing practice accurately and thoroughly in compliance with standards of care. Facilitates patient throughput on shift basis. Contributes to the environment of care to support clinical colleagues, patients, and families. Advances clinical competence in nursing practice. Projects a professional image to colleagues and communicates with caring and respect. Incorporates ethical principles into decision-making for patients and families. Encourages a spirit of scholarship, inquiry, and life-long learning for self and others. Participates in shared decision-making through nursing department committees and unit initiatives.
