This role offers a flexible remote work setting, enabling social workers to manage their scheduling effectively while maintaining essential support. The work hours will primarily be Monday through Friday from 8:00 AM to 4:30 PM, with occasional evenings or weekends as needed based on client requirements. Key Responsibilities\n

As a Licensed Master Social Worker at The Enlightened Mind Counseling Center, your day-to-day responsibilities will include:

• Developing tailored care plans in collaboration with clients and their families to address individual goals, challenges, and preferences.

• Providing emotional support through counseling sessions, guiding seniors through the recovery process and associated life transitions.

• Advocating for clients to secure their rights and access to critical community resources, including healthcare services and support groups.

• Coordinating with interdisciplinary teams, including physicians, nurses, and nutritionists, ensuring alignment in care strategies and services delivered.

• Monitoring clients' conditions through regular virtual check-ins, providing timely interventions for any emerging concerns or safety issues.

• Facilitating family meetings to improve communication, address concerns, and resolve conflicts regarding care approaches.

• Keeping accurate and up-to-date documentation of client progress, care plans, and interactions within our electronic health records system.

In this role, you will primarily be working with seniors experiencing substance use disorders who require support during their detoxification process. Compensation and Benefits\n
