Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, taking into consideration emotional, mental, and social influences on recovery.

• Collaborate with seniors and their families to formulate personalized care plans that reflect individual goals and preferences, and adjust plans based on client progress.

• Provide ongoing counseling, facilitating emotional support sessions that help clients manage stressors associated with detoxification and recovery.

• Advocate effectively for clients' rights, ensuring they have full access to essential services and community resources that aid in recovery.

• Work closely with our multidisciplinary team, including healthcare professionals, nutritionists, and care managers, to ensure holistic treatment plans are executed.

• Conduct regular check-in appointments to monitor client progress, address changes in situation or needs, and ensure safety within their living environments.
\n
• Facilitate family meetings and provide conflict resolution when necessary, enhancing communication and support among family members involved in client care.

• Educate clients and their families regarding available community resources, support groups, and proactive programs that foster social engagement.

• Maintain accurate documentation of assessments, treatment plans, progress notes, and all client interactions in our electronic health record system.

• Remain informed on best practices within the field of social work and engage in continuous professional development opportunities.

This role primarily serves the senior population, many of whom may face unique challenges associated with aging, such as isolation, health issues, and navigating the recovery process from addiction. Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current state licensure as a Licensed Master Social Worker (LMSW).

• A minimum of two years' experience in a social work capacity, ideally in a community or geriatric setting.

• Strong understanding of social work principles, particularly concerning aging-related issues and community resources.

• Demonstrated success in assessment, counseling, and interpersonal problem-solving skills.

• Exceptional empathy and ability to build rapport with seniors and their families.

Our compensation package offers a competitive annual salary that ranges from $60,000 to $100,000, commensurate with experience and qualifications.
