Key Responsibilities\n

• Conduct comprehensive assessments to identify the psychosocial needs of clients entering the detox program, taking into account their emotional, mental, and social factors.

• Develop and implement individualized treatment plans in collaboration with clients, their families, and the multidisciplinary team, tailored to meet their specific needs and recovery goals.

• Provide ongoing counseling and emotional support to clients, guiding them through the challenges of detoxification, and improving their coping strategies.

• Advocate for clients' access to vital community resources, leveraging available support systems that enhance their recovery experience.

• Maintain thorough documentation of client interactions, treatment progress, and adjustment of care plans within the electronic health records (EHR) system to ensure compliance and continuity of care.

• Conduct regular follow-up sessions to monitor clients’ progress, address any concerns, and adjust care plans as necessary to align with their evolving needs.

• Facilitate family involvement in the treatment process, helping to bridge communication gaps and provide guidance on supporting their loved ones through recovery.

• Participate actively in team meetings and case reviews to promote collaborative treatment approaches and share insights on best practices.

• Stay updated with current trends in social work practices, ethical standards, and recovery methodologies to enhance service delivery.

Patient Population / Client Focus\n

This role primarily focuses on individuals undergoing detoxification from substance use disorders. You will work closely with clients who are often experiencing emotional turmoil, as well as accompanying challenges related to family dynamics and social support systems. Your role is critical in providing holistic care that addresses not only substance use issues but also the undercurrents of mental health concerns, ensuring a comprehensive recovery approach.

Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited institution.

Compensation and Benefits\n
