Key Responsibilities\n

As a Licensed Professional Counselor at Congruent Health Center, your responsibilities will include:

• Conducting comprehensive assessments to effectively address the varied needs of each client.

• Regularly communicating with a multidisciplinary team including supervisors, direct care staff, and family members to track client progress and adjust treatment plans as necessary.

• Creating and managing a tentative schedule of appointments, home visits, and observations to assess and support treatment outcomes.

• Coordinating essential home visits and chart reviews as part of the care plan, ensuring a thorough analysis of the client's situation.

• Implementing treatment plans accurately and effectively, ensuring adherence to program goals and individual client needs.
\n
• Participating in Interdisciplinary Team Meetings to discuss cases, share insights, and refine treatment approaches.

• Documenting progress through detailed monthly and quarterly notes to maintain clear records of client developments.
\n
• Organizing training sessions for staff to ensure that all team members are competent in implementing treatment plans and recommended documentation procedures.

• Providing follow-up support to clients through interviews and home visits to reinforce treatment plans and ensure continuity of care.

• Maintaining accurate records that are readily accessible to authorized parties involved in a client’s care.

• Acting as a liaison with the broader mental health community to promote client and organizational support for development goals.

• Assisting in creating instructional materials aimed at the continual improvement of program implementation.

Qualifications\n

The ideal candidate will have:

• An active and unrestricted LPC license issued in the state of California.

Compensation and Benefits\n
