Float Social Worker - Nursing Home
Located throughout New York City, NY Salary Up To $85K Per Year; Based On Experience Social Worker Job Description:- Assist new residents and their family in the initial adjustment phase, as well as in the ongoing transition to institutional living
- Provide information to the resident, and/or family member, on residents' rights and responsibilities
Qualifications:
- Must have prior nursing home / long-term care experience
- Previous experience as a Social Work Director
- Strong knowledge of discharge planning, care coordination, and resident advocacy
- Experience working with interdisciplinary teams
- Familiarity with state and federal regulations related to long-term care
