Full-Time In-Home Hospice RN Case Manager

Optum

Caretenders Hospice, part of the LHC Group and Optum family, is hiring an RN Case Manager for in-home hospice care across a 50-mile radius in Ohio. You will deliver the Plan of Care, collaborate with other professionals, and ensure compassionate, evidence-based nursing support for patients and families. Standard hours are Monday–Friday, 8:00 AM–4:30 PM with on-call rotation for evenings and weekends. Current Ohio RN licensure is required, with travel within the coverage area a must. #J-18808-Ljbffr Optum
