Physical Therapist Case Manager: Home Health

We have multiple opportunities for Physical Therapists in Northern California with potential sign‑on and relocation packages. What you’ll do as a Home Health Physical Therapist Case Manager Perform an evaluation in the home and develop a care plan, collaborating with PTAs to follow the plan. Help patients with mobility, movement, and regain their independence. Create detailed treatment and care plans for each patient. Communicate and coordinate with patients’ physicians to ensure quality care. Evaluate and report patients’ progress to MD. Competitive compensation with an achievable incentive bonus program. To be a success as a Physical Therapist Current unrestricted CA Physical Therapist license. Current BLS/CPR certification. Valid CA driver’s license. Knowledge of local community resources. Understanding of individual development and human behavior as it relates to illness/injury and cultural influences on health care. Knowledge of state and federal home‑care regulatory guidelines preferred. Experience in home health care highly preferred. Experience with high acuity patients a plus. Experience working with an interdisciplinary team highly desired. Case Management fundamentals highly desired.
