Acute Inpatient Registered Nurse - RN

Overview PURPOSE AND SCOPE: The Inpatient RN provides direct patient care in accordance with company policies and procedures including FMCNA compliance programs and contracted hospitals’ policies and procedures. Areas of practice include, but are not limited to, hemodialysis, peritoneal dialysis, continuous renal replacement therapies, apheresis, and ultrafiltration. The Inpatient RN partners with the hospital to provide quality care and a positive patient experience. Responsibilities Under limited supervision, apply knowledge to perform a broad range of direct patient care in multiple modalities: hemodialysis, peritoneal dialysis, ultrafiltration, continuous renal replacement therapy, and apheresis. Supervise and direct Patient Care Technicians and clerical staff; provide direction and education to ICU staff for CRRT. Deliver direct patient care in the Inpatient Renal Replacement and Apheresis Services setting and administer the prescribed treatment techniques for assigned patients across all modalities. Assess patients’ responses to treatment and adjust the plan as indicated by appropriately credentialed physicians. Communicate problems or concerns to the Inpatient Program Manager, appropriately credentialed physician, patient hospital primary nurse, and others as indicated. Deliver safe, effective care in a timely and efficient manner; provide organized, concise shift reports. Adjust shift patient schedules to meet needs of more critical patients based on labs, MD orders, or patient assessments. Implement, administer, monitor, and document patient responses to prescribed intradialytic transfusions; notify physicians and blood suppliers of adverse reactions. Collaborate with physicians and other healthcare team members to interpret, adjust, and coordinate daily patient care plans to ensure continuity of care. Initiate communication with FMS facility nursing staff and appropriate hospital personnel as needed. Document all treatment information accurately in the patient record (ACES) and hospital record as needed. Clean and disinfect dialysis equipment and surrounding areas between treatments according to policies and procedures. Prepare for dialysis treatment and document preparation where appropriate; perform pre-treatment dialysis machine alarm testing. If applicable, initiate the Solution Delivery System; operate related equipment safely and troubleshoot as needed. Conduct dialysis/apheresis machine safety tests, calibrations, dialysate testing, machine safety tests, and internal/external disinfection; document per policy. Participate in QAI activities and staff meetings; participate in staff training and orientation as assigned. Serve as a subject matter expert to hospital staff for dialysis-related patient/equipment complications. Report all new or unusual incidents to the inpatient manager and other parties per Adverse Event Report requirements. Review and comply with the Code of Business Conduct and all applicable policies, procedures, laws, and regulations; assist with projects as assigned; perform other duties as needed. Physical Demands and Working Conditions The role requires travel between assigned hospitals and facilities, on-call rotation including night, day, weekends, and holidays. Potential exposure to infectious material. Direct patient care activities with heavy lifting, moving patients/equipment (>200 lbs with assistance) and handling chemical and water solutions up to 30 lbs, up to 5 feet high. Reasonable accommodations may be made to enable performance of essential functions. Supervision Provides supervision and direction of Patient Care Technicians (Certified Clinical Hemodialysis Technicians, CCHTs) and clerical staff; provides direction and education to ICU staff for CRRT. Education and Certification Current appropriate state licensure. Maintain updated knowledge in nephrology and inpatient care across dialysis, apheresis, and ultrafiltration modalities; meet state practice requirements. Certification in Nephrology Nursing, Apheresis Nursing, or both is highly encouraged. Experience and Required Skills One year RN experience required; two specified nephrology tracks: at least 6 months nephrology experience before independent work with dialysis RNs; at least 6 months nephrology experience before independent work with apheresis RNs. Strong assessment skills for critically ill patients; ICU experience preferred but not required. Successful completion of training related to ESRD patient care (Progressive Renal Education Preparation). BLS CPR required; ACLS CPR preferred but not required. Demonstrated organizational and time-management abilities; commitment to ongoing renal/nephrology education; leadership skills to supervise others (PCTs, CCHTs, clerical staff). Successful completion of Nurses Technical Training.
