This role is fulfilled within a remote work setting, allowing for flexibility and convenience while maintaining essential connections with clients and team members. While most communications occur via virtual platforms, occasional in-person interactions may be required to facilitate comprehensive care and discharge planning at the Hospice Center located in Macon, Georgia. Key Responsibilities\n

• Act as a vital liaison between families and outpatient providers, ensuring seamless communication and understanding of clients' care plans.

• Interface with insurance companies for pre-certification processes and utilization management, facilitating necessary authorizations for client services.

• Engage actively in discharge and aftercare planning, coordinating resources to ensure continuity of care and identify further support options for clients.

• Provide therapeutic meal support during sessions, modeling healthy relationships with food while addressing any psychosocial barriers related to nutrition.

• Deliver empathetic, safe, and affirming care, tailoring your approach based on the unique intersecting identities of each client to enhance their overall care experience.

This position primarily involves working with older adults in a hospice setting, addressing mental health concerns as clients navigate complex emotional and psychological challenges associated with end-of-life issues. Compensation and Benefits\n
