Key Responsibilities\n

• Develop, implement, and coordinate comprehensive support plans tailored to each individual’s needs to assist clients in overcoming emotional difficulties and dependencies.

• Conduct in-depth interviews with clients and assess their psychological, social, and emotional conditions to identify strengths, weaknesses, and barriers to recovery.

• Set specific, measurable, and personalized goals for treatment, ensuring they are aligned with clients' overall care plans.

• Continuously monitor clients’ progress, adjusting treatment strategies and methodologies based on their individual responses to interventions.

• Provide guidance and counseling during regular sessions, delivering critical information on resources and strategies for emotional and psychological resilience.

• Maintain meticulous records regarding each client’s status and progress, ensuring compliance with relevant regulations and policies.

• Participate in case conferences, sharing insights and collaborating with other professionals to develop holistic care plans.

• Serve as a key worker who maintains consistent communication and coordination with clients’ families and external service providers.

• Facilitate referrals to community resources, enabling clients to access additional services necessary for effective recovery.

You will primarily support clients in a hospice setting, where delivering emotional and psychological care plays a pivotal role in their quality of life. Qualifications\n

• A Bachelor’s degree in Social Work (BSW) or a related field is required; a Master’s degree in Social Work (MSW), psychology, or sociology is preferred.

Compensation and Benefits\n

We offer a competitive salary ranging from USD 80,000 – 100,000 annually, commensurate with experience and qualifications. Additionally, our benefits package includes:
