This full-time position offers the flexibility of a remote work setting while serving within the framework of a Hospice Center. The remote environment is one where you can effectively collaborate with interdisciplinary teams while having the autonomy to manage your caseload. Although the role is largely virtual, it is vital that you can accommodate periodic in-person meetings or trainings as necessary, to ensure alignment with our organization’s mission and patient care standards.

Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify each patient's mental health concerns, strengths, and available resources, ensuring a comprehensive understanding of their needs.

• Deliver individualized, family, and group therapy using evidence-based therapeutic methods tailored to the unique circumstances of the patient population.

• Implement specialized treatment modalities including cognitive-behavioral therapy, solution-focused therapy, and trauma-informed interventions, ensuring that care is effective and compassionate.

• Develop, review, and update personalized treatment plans that include specific, measurable goals and strategies to promote optimal patient outcomes.

• Collaborate closely with psychiatrists, psychologists, healthcare providers, and other professionals to guarantee cohesive, holistic care for all clients.

• Facilitate crisis intervention, risk assessment, and safety planning tailored to individual patient needs, ensuring proactive measures are in place.

• Maintain comprehensive and detailed records of patient interactions and treatment interventions in compliance with ethical and legal standards, utilizing electronic health record systems effectively.

• Organize and lead group therapy sessions, psychoeducation classes, and support groups that serve diverse client demographics, promoting engagement and emotional support.

Patient Population / Client Focus\n

Your role will predominantly focus on patients within the hospice setting, who may be facing terminal illnesses or chronic health conditions. This includes providing emotional support to individuals grappling with grief, loss, and anxiety. You will work closely with families to address their needs, facilitate coping mechanisms, and promote resilience in the face of challenging emotional landscapes.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
