Perform start-of-care (SOC) and ongoing reassessments, developing and updating individualized care plans with the care team. Conduct and document home health visits per plan of care, monitor patient status and safety indicators, and elevate changes promptly. Educate patients and families on disease management, medications, safety at home, and self-management strategies to support adherence. Maintain accurate documentation in the electronic chart, ensure regulatory compliance, and coordinate with the interdisciplinary team to optimize outcomes. Requirements Active Pennsylvania RN license in good standing 2+ years of home health nursing experience Start of Care (SOC) and revisit experience required RN degree (Associate or Bachelor’s) from an accredited program Ability and willingness to travel to patient homes within service ZIP codes: 19040, 19044, 18929, 18974, 18976, 19002, 18954, 18966, 18901, and to work in a home health setting Benefits and Support Guaranteed hours Local Contract (Nursing) program 24/7 recruiter and travel support Dedicated support throughout the assignment Assignment length: 12 weeks Please note: Pay rates and guaranteed hours are estimates and may vary.
