Work Environment

You will work onsite at our state-of-the-art Rehabilitation Center in Santa Rosa, where modern facilities and advanced equipment create an optimal space for patient recovery. Your typical work hours will be Monday to Friday, from 9 AM to 5 PM, allowing for a balanced work-life structure. Key Responsibilities

• Conduct thorough evaluations of each patient's physical condition to establish a customized treatment plan that addresses both immediate and long-term rehabilitation goals.
• Implement a variety of evidence-based therapeutic techniques, including manual therapy, therapeutic exercises, and modalities such as hydrotherapy and ultrasound, to facilitate patient recovery.
• Regularly monitor and assess patient progress, adjusting treatment plans as necessary to ensure that patient needs are met and goals are achieved.
• Guide patients through their rehabilitation journey by providing educational resources, creating individualized home exercise programs, and fostering empowerment in managing their recovery.
• Maintain detailed progress notes and documentation consistent with clinic protocols, ensuring clear communication and continuity of care.
• Engage in effective teamwork with other healthcare professionals and staff to adopt an integrated approach to patient treatment.
• Build and nurture strong relationships with patients, providing moral support, motivation, and encouragement throughout their rehabilitation.

Patient Population / Client Focus

You will have the opportunity to work closely with patients of all ages, including children and the elderly, allowing for a dynamic and fulfilling practice. You will play a pivotal role in guiding these individuals from their initial assessment through to achieving functional independence and improved quality of life.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A valid state licensure or eligibility for licensure as a Physical Therapist in California.
• A solid understanding of human anatomy and physiology as it relates to physical therapy best practices.
• Excellent interpersonal skills, enabling effective communication with patients, their families, and your healthcare team.
• Proficiency in both independent and team-based work environments, exhibiting adaptability and collaborative spirit.

Preferred Experience

• Minimum of two years of relevant experience in settings such as outpatient therapy or rehabilitation care.
• Experience in pediatrics or acute care settings is beneficial, although not mandatory.
• Familiarity with electronic medical records (EMR) systems to streamline documentation and patient tracking.

Compensation and Benefits
